We are looking for a relationship‑oriented Community Partnership Liaison who thrives on connecting with people, building trust, and representing a mission that truly makes a difference while elevating our brand presence across the community. This is an existing vacant position.

What You’ll Do

Build Trusted Relationships
  • Develop warm, long‑lasting connections with referral partners, healthcare professionals, and community leaders.
  • Maintain regular, friendly outreach to ensure partners feel appreciated and confident in referring families to us.
  • Position Warm Embrace as the trusted, go‑to partner for senior care solutions.
Lead Community Engagement
  • Represent Warm Embrace at community events, health fairs, networking meetings, and conferences.
  • Serve as a positive, approachable ambassador who strengthens our presence and reputation.
Maintain a Strong Community Presence
  • Visit referral sources regularly to nurture relationships and keep our services top‑of‑mind.
Organize and Execute Marketing Efforts
  • Plan and coordinate a thoughtful calendar of outreach activities, events, and campaigns.
  • Create marketing initiatives that resonate with families, healthcare partners, and community organizations.
Support Families Through the Intake Process
  • Meet with seniors and their families to understand their needs with empathy and clarity.
  • Recommend care solutions that prioritize safety, reduce family stress, and honour the client’s dignity.
Support Client & Family Satisfaction
  • Respond to concerns with patience and understanding.
  • Ensure clients and families feel heard, supported, and cared for throughout their journey with us.
Track Progress & Share Insights
  • Record interactions, monitor referral trends, and evaluate campaign effectiveness.
  • Share insights that guide future outreach and strengthen team collaboration.
Collaborate with the Office Team
  • Work closely with internal staff to ensure smooth transitions from inquiry to service.
  • Contribute to a positive, supportive team environment where everyone feels valued.
Participate in On‑Call Rotation
  • Provide after‑hours support when needed, helping caregivers and families navigate urgent situations with calm and care.

What You Bring

Experience
  • 3–5 years of experience in healthcare or social services (social work, nursing, case management, etc.)
Skills
  • Exceptional interpersonal and communication skills; able to connect with people from all backgrounds.
  • Strong understanding of the needs of seniors and their families in Waterloo Region and Wellington County.
  • Ability to plan, organize, and follow through on multiple initiatives at once.
  • Comfortable guiding families through emotional decisions with empathy and clarity.
  • Proficient with CRM systems, marketing tools, and Microsoft Office.
  • Organized, dependable, and able to balance independent work with team collaboration.

Total Compensation
  • $55,000—$85,000+ comprised of hourly pay plus bonus incentives and results-based pay.
